2019 Joint Affiliate Annual Disclosure Due February 28

As in prior years, the Joint Affiliates* have partnered to conduct an annual disclosure survey. All Northwestern Medicine employed physicians, physician consultants, medical and doctoral staff, and advanced practice providers are subject to one or more of the Joint Affiliates' respective policies regarding conflicts of interest and business integrity. These policies require annual disclosure of certain external professional relationships and/or activities. In addition, those individuals conducting sponsored research are also required (by both federal and institutional policies) to transactionally disclose external activities as directed by their respective institutions.
On February 1, you received the link to the survey and your login instructions. The 2019 Joint Affiliate Annual Disclosure Survey covers external professional relationships and/or activities for calendar year 2018. Completion of the annual survey is required by February 28.
